These appointments have their own logistics. Visiting hours, front desk sign-in, a nurse who needs to be asked before anyone enters. We work inside those constraints, and we always confirm the signer is alert and acting freely before anything is signed. If they are not, we stop and reschedule, because a document signed by someone who could not consent is worse than no document.
